Complete Pre-Check-In
Once your first visit has been scheduled, you will receive a confirmation email and text message from our digital check- in platform, JaneAPP. This message will include a link to setup your account which will include a set of digital forms that must be completed before we can see you.
If you need assistance with your pre-visit forms? No problem! Call us or please arrive 20 minutes prior to your appointment time and our staff can answer your questions.
The Forms you need to complete are Profile & Consent Form, Insurance Verification Request and the * Medical History Form.
If you finish your pre-visit, please arrive to the clinic at least 15 minutes prior to your appointment time.
Cancellations
If you need to cancel or reschedule your appointment, please call our scheduling team as soon as possible at 512-360-9480. We request at least 24 hours notice if you will need to cancel your appointment. We reserve the right to charge $75 for missed appointments or appointments canceled with less than 24 hours’ notice.
